• Mail a check or money order for $25.00 to PO BOX 69 Lebanon Junction, Ky 40150
• Please include how you would like to receive the report; by mail or email
• You must include the report number
• NO REQUEST FOR REPORTS WILL BE TAKEN OVER THE PHONE; ONLY MAIL
